First visit
During a first visit, it is important to fill out a form giving us all the relevant information on your state of health. To speed up the service, we invite you to fill it out and send it to us by email. Please bring a list of your medications if necessary.

Dental insurance
At the Bélanger Paré and Associates Dental Center we offer free electronic service (internet) for your dental insurance plans. Insurance forms that can not be sent over the Internet will be filled out free of charge and sent directly to your insurer.
The treatment plan (cost evaluation) for the various treatments are also completed free of charge and are added to the many services offered at the Bélanger Paré and Associates Dental Center.
We want to remind you that insurance coverage varies depending on the agreement signed between the insurance company and your employer. It is illegal to overcharge your insurance or to claim the difference. It is therefore your responsibility to inform yourself about your coverage and the maximum amount allocated per year.
Payment terms accepted
We accept Visa and Mastercard credit cards as well as debit cards. In addition, you can benefit from the DentoPlan Dental Financial Plan.
